Melbourne, Geelong & Ballarat
Looking for support that truly fits your needs? Loretta can help you explore the options available through Nextt, whether you’re looking for the right living arrangement, support team or a more personalised approach. She’ll take the time to understand what matters to you, explain what Nextt can provide and help you find the option that best suits your needs.
Loretta has worked in the disability sector for around 14 years, with experience across disability services and the NDIS. She understands that finding the right support is about more than securing a place to live or meeting someone’s support needs. It’s about understanding the person, what matters to them, and bringing together the right support, people and environment to help them feel genuinely settled and valued.
It’s more than just finding someone a bed. It’s about finding the right fit for the person.
— Loretta
Understand what the right fit looks like
Loretta starts by getting to know the person, their needs, funding and what matters to them. This helps identify the type of home, support and people that may be the right fit.
Take the conversation forward
Loretta can connect directly with the participant and their family to explore the options in more detail, while keeping the Support Coordinator involved without adding unnecessary back and forth.
Work towards a home where you can belong
Loretta looks beyond simply filling a vacancy, helping people find compatible housemates and a support team that can help them settle in and enjoy life in their new home.
Loretta can help you explore the range of support available through Nextt across Melbourne West, Geelong and Ballarat, and work out what may be right for your circumstances.

Yes. Whether you require Supported Independent Living in standard housing or Specialist Disability Accommodation, our team works closely with you to identify the best living arrangements. We regularly add housing options and can source based on your requirements — get in touch or visit our SIL and SDA Vacancies page for more details.
Supported Independent Living (SIL) refers to the support services — providing ongoing assistance with daily tasks to help individuals live more independently, in shared or individual living spaces. Most people with SIL funding live in standard community housing. Specialist Disability Accommodation (SDA) refers to purpose-built housing designed for individuals with high support needs who cannot access standard housing. Features can include ceiling hoists, assistive technology, strong walls, emergency power, and adjustable kitchen counters.
Eligibility for SIL depends on your NDIS plan and individual support needs. If you require significant daily assistance and aim to build your independence, SIL funding may be included in your plan. You’ll start with your housing goal, then complete a NDIS Home and Living application with as much supporting evidence as possible.
NDIS hospital discharge planning is the process of coordinating all the supports, accommodation and services a participant needs to leave hospital safely. For participants with complex needs, this includes confirming NDIS funding, securing suitable housing, recruiting and training support workers, arranging equipment, and ensuring all parties are working together toward the same plan.
Ideally at the time of admission, or as soon as it becomes clear that the participant’s support needs may change as a result of their hospital stay. Starting early is critical — organising funding approvals, housing, staffing and equipment can take weeks or months, particularly for participants with complex needs.
Confirmed accommodation, approved and funded NDIS supports, trained support workers, essential equipment, medications, and a clear communication plan between the allied health team, disability support provider, support coordinator and family. Supports need to be ready in practice, not just approved on paper.
Nextt supports participants with complex needs through the full hospital to home transition. We engage early with hospital teams and support coordinators, coordinate accommodation, workforce readiness and supports in parallel, and provide clinical oversight from day one. Whether a participant needs MTA as a bridge or is moving into long-term SIL, we are here to help.
Our team helps assess your preferences and needs, matching you with suitable accommodations and support plans. Nextt regularly adds properties and can help you find options tailored to your needs. Contact us to explore more.
