Courtney is a registered nurse with almost 18 years’ experience in the disability sector, with a background in acquired disability, acquired brain injury and rehabilitation. Her long-standing commitment to disability support, combined with experience in education and hospital-to-home transitions, gives her a strong understanding of the clinical and practical considerations involved in establishing the right support after hospital and in other complex situations.

Courtney can help work through the specific support requirements, particularly where someone has complex needs or is transitioning from hospital. Her clinical background helps her understand the care plans, documentation and practical considerations needed to establish the right support around the person.
Coordinate your transition into supports
Courtney will guide you through the referral process, drawing on her relationships with hospitals, SDA providers and Support Coordinators to help bring the right people together. She’ll coordinate the key information and next steps, keeping everyone informed at each stage throughout the transition.
Find the right support team for you
Where appropriate, Courtney can arrange a video meet and greet so participants can meet potential support workers and have a say in who supports them. She understands that knowing who will be supporting you can make a difficult transition easier, particularly when you’re welcoming new people into your home.
Courtney stays in contact after support starts, following up with the participant or Support Coordinator to check whether things are going as planned and whether there are any concerns that need to be addressed.

Supported Independent Living (SIL) refers to the support services — providing ongoing assistance with daily tasks to help individuals live more independently, in shared or individual living spaces. Most people with SIL funding live in standard community housing. Specialist Disability Accommodation (SDA) refers to purpose-built housing designed for individuals with high support needs who cannot access standard housing. Features can include ceiling hoists, assistive technology, strong walls, emergency power, and adjustable kitchen counters.
Eligibility for SIL depends on your NDIS plan and individual support needs. If you require significant daily assistance and aim to build your independence, SIL funding may be included in your plan. You’ll start with your housing goal, then complete a NDIS Home and Living application with as much supporting evidence as possible.
NDIS hospital discharge planning is the process of coordinating all the supports, accommodation and services a participant needs to leave hospital safely. For participants with complex needs, this includes confirming NDIS funding, securing suitable housing, recruiting and training support workers, arranging equipment, and ensuring all parties are working together toward the same plan.
Ideally at the time of admission, or as soon as it becomes clear that the participant’s support needs may change as a result of their hospital stay. Starting early is critical — organising funding approvals, housing, staffing and equipment can take weeks or months, particularly for participants with complex needs.
Confirmed accommodation, approved and funded NDIS supports, trained support workers, essential equipment, medications, and a clear communication plan between the allied health team, disability support provider, support coordinator and family. Supports need to be ready in practice, not just approved on paper.
